We’ll arrange an informal conversation to understand the individual’s needs and aspirations. This helps us learn more about their ideal living environment—whether that’s independent living.

Holistic Needs Full Assessment
We carry out a comprehensive, person-centred assessment to get a full picture of health, daily living, and emotional well-being needs. This also helps us identify any potential risks and how to manage them safely.

Tailored Support Proposal
Once funding is confirmed through the local authority or ICB, we create a detailed support proposal outlining how we can meet the person’s needs—always shaped by their goals and preferences.

Planning for Success
We design a custom support framework, from selecting the right support staff to working with trusted housing partners to secure safe and suitable accommodation that feels like home.

Meet Your Support Team
We introduce the support team gradually, ensuring everyone feels comfortable and confident. Observations during this phase help us fine-tune our approach to meet individual preferences and reduce stress during the transition.

Moving into a New Home
We’re here to make the move smooth and empowering. From managing tenancy agreements to furnishing and settling in, we work closely with our housing partners and the person we support every step of the way.

Support Begins
With everything in place, your support officially begins—whether that’s in a new home, shared living, or in the community through our outreach services. We’re right by your side, from day one.

Ongoing Support & Progress Reviews
Six weeks after support begins, we carry out a progress review to ensure things are going smoothly. We then hold regular check-ins and a full annual review to celebrate achievements, address challenges, and adapt support to evolving needs.
